This happened to me once in the past and again today about 1/2 hour ago. I kept entering the wrong rcon password ( 5 Times ) and hlsw ip banned me from having control of my game server remotely.
Solution: Restarting your game server will erase the ip banned from hlsw and you can re-enter your rcon password to remotely control your game server.
